Customer Declaration / Authorization

1. I desire to voluntarily make use of the services which Kadamba and its affiliates (collectively referred to as “Kadamba”) offer. Said services include Ayurvedic nano medicines with Gold/Silver nano particles (“the services”).
2. I understand that every attempt shall be made to minimise the existing risks that are inherent in the services through the use of proper equipment and safe facilities.
3. I understand that the aforementioned risks cannot be eliminated completely.
4. I realise that by requesting the services, I could possibly incur adverse effects.
5. I agree to accept responsibility for all necessary charges incurred by any hospitalisation or treatment which I, or my minor child, may receive pursuant to this authorisation.
6. I hereby confirm that I am voluntarily requesting the services with full knowledge of the potential danger which they present in any form.
7. I agree to release and hold harmless Kadamba as well as their officers, directors, employees, and agents from liability for bodily injury, property damage, or death arising from my or minor child’s engagement in the services.
8. I hereby give this release and indemnification in exchange for the opportunity for me/minor child to make use of the services.
9. By signing this agreement, you agree to receive email correspondence from Kadamba in respect of advertisement of future events and/or information newsletter format. Should you wish to unsubscribe, you may do so at any time by clicking “unsubscribe” at the foot of received correspondence.
10. This indemnity shall endure from date of signature hereof until such time as my minor child no longer engages in the services.
11. I confirm that I am 18 (eighteen) years of age or older and that I have read the above declaration/authorisation.
12. I confirm that the information contained herein is true and accurate.
